All other Knox and Lincoln County residents:
Send your checklist as an email attachment to louisa@knox-lincoln.org. You may also print and mail the checklist to our office (893 West St Ste 103, Rockport, ME, 04856).
After you submit your checklist:
Our staff or partner organization staff will contact you to schedule visit to your property to review your checklist with you. One complementary 1-hour site visit is provided for all program participants. Any additional time on site is charged standard technical service rate. A follow-up report is also provided free of charge.

This checklist is for all properties used for a residence, public access or school. If the property has 2-5 acres of woodlands, please also submit the Forested Lands checklist with this checklist. If there are more than 5 acres of woodlands, please certify those separately. The PDF has blanks you can fill and save on your computer, or you can print and write on the checklist.
